Miss-codes - A question

I'm intrigued as to why certain aircraft operate with miss-codes!  Ryanair for example have had several miss-coded HEX codes.  On certain airframes the operators seem unable to "get it right".

If the aircraft industry is moving towards using ADSB and HEX code identification why does the CAA/FAA etc put up with these errors.  So, why aren't the operators taken to task to get the codes correct for the airframe??
